Vital Records


BIRTH
Birth records may be obtained from:
* The city or town in which the mother was living at the time of birth

*The Maine Dept. of Health and Human Services Office of Vital Records in Augusta also maintains records from 1923 to the present.  Copies of these birth records can be ordered online at http://www.maine.gov/dhhs/bohodr/recorder.htm 
*The Maine State Archives also maintains older vital records (1892-1922).  Copies of these birth records can be ordered by phone (207-287-5795); mail (Maine State Archives, 84 State House Station, Augusta, ME 04333-0084) or email at anthony.douin@maine.gov

For copies of birth records issued by the Town of Gray, the fee is $10 for the first certified copy, and $5 for each additional copy.  We accept cash, checks and money orders—checks and money orders should be made payable to the “Town of Gray”.   

Requests by mail should be directed to the Town Clerk at 6 Shaker Rd., Gray, ME 04039.  All mail requests must include a self-addressed, stamped business-sized envelope and the:
        1.  name on the birth record
        2.  date of birth
        3.  town of birth
        4.  names of the parents—please include the mother’s maiden name
In addition, you must also state your relationship to the name on the birth record being requested.  Please include a daytime telephone number where you may contacted, in case there are any questions.


MARRIAGE
Marriage certificates may be obtained from:
* The town in which the bride and/or groom resided at the time of marriage

*The Maine Dept. of Health and Human Services Office of Vital Records in Augusta also maintains records from 1923 to the present.  Copies of these marriage records can be ordered online at http://www.maine.gov/dhhs/bohodr/recorder.htm 
*The Maine State Archives also maintains older vital records (1892-1922).  Copies of these marriage records can be ordered by phone (207-287-5795); mail (Maine State Archives, 84 State House Station, Augusta, ME 04333-0084) or email at anthony.douin@maine.gov

The Maine State Archives has two databases available online as an index to original records: Marriages (1892-1966 & 1976-1996) and Deaths (1960-1996).
