Vital Records: Marriage Record Request

• Open the marriage record request form (PDF) and complete all sections.  
• Submit either in person or by mail to Town of Gray, 24 Main Street, Gray, ME 04039.  
• If you are mailing your request, be sure to include copies of identification documents as described.
• Records are $15 for the first certified copy and $6 for each additional copy of the same record purchased at the same time.
• We accept cash, checks and money orders—checks and money orders should be made payable to the “Town of Gray”. 
• If you are mailing your request, please also include a self-addressed, stamped envelope.

Marriage records may also be obtained from:

• The Maine Dept. of Health and Human Services Office of Vital Records in Augusta also maintains records from 1892 to the present.  Copies of these records can be ordered online.
• The Maine State Archives issues non-certified copies of records prior to 1892.
