Honor Roll Nomination

The Town of Gray Honor Roll is intended to be awarded to persons who, during the course of their lifetimes, demonstrated exceptional dedication, service or other contribution to the Town of Gray. Nominations are only acceptable for persons deceased a minimum of five years at the time of   application. Honoree is required to have lived in Gray at some point in their lives.

The Honor Roll Application is also available in PDF format if you prefer to print and mail/deliver your application.  When submitting an application by mail or in-person delivery, supporting materials, including photographs, must be submitted with the application package.

APPLICATION REQUIREMENTS

  • The Town of Gray Honor Roll is intended to be awarded to persons who, during the course of their lifetimes, demonstrated exceptional dedication, service or other contribution to the Town of Gray. Nominations are only acceptable for persons deceased a minimum of five years at the time of application. Honoree is required to have lived in Gray at some point in their lives.
  • Applications are encouraged and accepted throughout the calendar year, but no later than October 1st. This application must be submitted to the Town Manager, who will record and date its receipt. A copy shall be forwarded to the Gray Historical Society for comment at their option.
  • Any person may sponsor a nominee. No relationship to the nominee is required. Please supply sponsor contact information below. Sponsor is not required to appear before the Town Council.
  • Fill in all information requested on this application. Paper or electronic submissions are accepted. This application will appear in the permanent Honor Roll book.
  • If possible, please supply/attach a photograph or image of the nominee.
  • Additional pages, supporting material and presentations may be supplied by the sponsor, and are encouraged to help ensure approval, but will not appear in the Honor Roll book.
  • Supply no more than fifteen words under the category of “In Recognition For”. The Town Council may choose to edit this summary.
  • No application materials will be returned, and none are guaranteed to be preserved. Please provide only copies of historic materials rather than originals. All application materials are part of the public record and become the property of the Town of Gray.
  • No monetary or other contributions of value, donations or reimbursements associated with any application will be accepted. No fees are associated with this application.
  • An affirmative vote of the Gray Town Council is required for a nominee to be named to the Town of Gray Honor Roll. Any Council vote shall take place in November or December from among the nominees for that calendar year. The Council may decline to approve any or all. Beginning in the year 2015, the Council may approve a maximum of two applicants in any given calendar year.
  • Suspension of any of the above rules requires four affirmative votes by the Town Council.
